HealthVisions Midwest is seeking nominations for individuals and organizations who are making a real difference in the community’s health. Do you know someone who goes above and beyond to promote well-being? Nominations are now open!

Community Health Worker Champion Award
This award is presented to an individual who exemplifies the role of a Community Health Worker. A CHW champion works to address health disparities through direct case management, education, or advocacy. They deliver culturally sensitive care and facilitate access to healthcare services within their communities. This CHW Champion serves as a role model for other community health workers. Their work helps reduce health disparities and fosters a healthier and more connected community, inspiring others to follow their lead. 

Health Visionary Award
This award is presented to a healthcare provider or practitioner who has demonstrated exceptional foresight, innovation, and leadership in advancing healthcare, public health, and wellness initiatives. This award honors those who have shown remarkable dedication, creativity, and excellence in addressing critical health challenges, driving positive change, and inspiring others to follow their lead in creating a healthier, more equitable future for all. 

Equity Visionary Award
This award is presented to an organization with an unwavering commitment to advancing health equity in our community while impacting countless lives through its initiatives, programs, and services. They have acted as a leader in the health field and facilitate opportunities for collaboration, innovation, and change. This award celebrates their outstanding contributions and leadership in promoting equitable healthcare access and improving community outcomes throughout NWI. 

HealthVisions Lifetime Achievement Award
This award is presented to an individual who has made a significant and enduring contribution to the field of health and wellness. It acknowledges their impact and legacy in improving the health and well-being of all individuals, especially those in underserved communities. Their dedication and passion for community, health, and service is not only inspiring but contagious and is in direct line with HealthVisions mission to help people live healthier lives throughout NWI.
