Health Visions of Fort Wayne is a community based health improvement organization. A ministry of the Poor Handmaids of Jesus Christ, since 1998.

Health Visions is committed to building stronger neighborhoods to ensure a healthier future in Fort Wayne. Bringing community partners together, and effectively linking existing resources, is a core principle of the organization.

HealthVisions is partnering with others to build the capacity of the community to reduce health disparities in identified Fort Wayne neighborhoods.

      • Genesis Outreach Inc offers affordable transitional and emergency shelter housing for persons and families who are homeless or at-risk of becoming homeless, with special emphasis given to persons who are overcoming alcohol and chemical dependencies, and the mentally ill.

  • Advocacy — Provide a voice for the poor and underserved
  • Education/Facilitation — Provide education and training of community teams and facilitation of partnerships and groups
  • Funding — Identify sources of funding for action and community projects

  • Management Support— Provide planning and management services to community health organizations including: Organizational Management, Finance, and Human Resources

Mission Statement

HealthVisions of Fort Wayne is a Faith-based Organization sponsored by the Poor Handmaids of Jesus Christ. We work in communities to:
  • Advocate for the poor and powerless population
  • Empower people to live a healthier life style
  • Nurture and foster leadership
  • Develop and facilitate partnerships
  • Address systemic community health needs
  • Promote good health and health education

Vision

To partner with people and organizations to create healthy communities by empowering people to live a healthy life.

Donate

  • $25 gift cards to serve as incentives for the participants of the Diabetes program.
  • $50 will provide a stipend for one student to help seniors for one month.
  • $150 will purchase a English-Spanish translation dictionary.
  • $300 will provide gas cards for (4) volunteers to transport seniors to the doctors and shopping for a month.
  • $400 will purchase 25 books for the participants of the Bienvenido Program.
  • $400 will train (2) facilitators for the Bienvenido Programs - A program that assists the Hispanic-Latino population.
  • $500 will provide volunteer training for Faith in Action for 10 churches, training 100 volunteers.
  • $750 will provide materials for 10 youth workers for special projects.
  • $3500 would provide training for 10 community volunteers to become Community Health Workers.
  • $5000 would provide free health screens for the Prevention Clinic.
  • $5000 provide Summer/Winter Lawn care services to 200 seniors for a year.
